Harrisonburg OB/GYN Releases Information on HIPAA Breach
Harrisonburg OB/GYN (the “Practice”) is providing notice of a breach of certain of its patients’ health information under the Health Insurance Portability and Accountability Act of 1996 and its implementing regulations (“HIPAA”). An ongoing investigation beginning on September 10, 2016, identified that on or about August 12, 2016, one of its physicians printed and impermissibly removed reports containing patient names, account numbers, addresses, phone numbers, and date and reason of last visit. The physician has since left the Practice. At this time, the Practice has notified all patients which it knows or reasonably believes to have been affected by this breach, and there are no additional steps patients should take to protect their personal health information. The Practice has been working diligently to mitigate the disclosure, including seeking return of the reports. To protect against further breaches, the Practice has limited employees’ ability to generate similar reports from its electronic health record software, and will routinely audit its software for impermissible activity.
